Wooden beads with teddy clip and co-ordinating beads made to order.  However, the more letters in a name, the less additional beads can be included as for safety reasons the length of these is limited.  
Never again drop a dummy out of the pram, as the clip is fixed to clothing or pram/car seat straps.
